Welcome to the Jamesville-DeWitt Central School District. The district is currently registering students in person and electronically. In-person registration is by appointment only. Contact Registrar Lisa Howe at 315-445-8300 to make an appointment. To register electronically, please read all instructions below. 

Required Documents

In addition to the registration forms, the following documents are required for registration:

  • Parent identification,
  • Proof of student’s birth,
  • Proof of residency, and
  • Custody papers, if applicable.

Please see below for acceptable documents for each of the above items.

Parent Identification

Identification of the parent(s), person(s) in parental relation of child:

  • Driver’s license, OR
  • Passport

If a driver’s license or passport is unavailable, other documentation identifying the child’s parent(s) or person(s) in parental relation may be provided, including but not limited to the following:

  • Affidavit indicating that they are the parent(s) with whom the child lawfully resides.
  • Affidavit indicating that they are the person(s) in parental relation to the child over whom they have total and permanent custody and control and describing how they obtained total and permanent custody and control, whether through guardianship or otherwise.
  • Documentation indicating that the child resides with a sponsor with whom the child has been placed by a federal agency.

Proof of Student’s Birth

  • Original birth certificate (i.e., a certified transcript of a birth certificate), OR
  • Baptismal certificate

If neither of these documents is available, then a passport may be used to determine a child’s age.

If a birth certificate, baptismal certificate or passport is unavailable, then other documentary or recorded evidence in existence for two years or more (excluding an affidavit of age) may be considered, including but not limited to the following:

  • Official driver’s license
  • State or other government issued identification
  • School photo identification with date of birth
  • Consulate identification card
  • Hospital or health records
  • Military dependent identification card
  • Federal, state or local agency documentation (i.e. social service agency, federal Office of Refugee Resettlement)
  • Court orders or other court-issued documents
  • Native American tribal document
  • Non-profit international aid agency and voluntary agency documentation

If the documentary evidence presented originates from a foreign country, the district may request verification of such documentary evidence from the appropriate foreign government or agency, consistent with the requirements of the federal Family Education Rights and Privacy Act (20 USC §1232g), provided that the student shall be enrolled and shall begin attendance on the next school day or as soon as practicable.

Proof of Residency

Proof of residency documentation may include, but is not limited to, the following:

  1. Residential lease or proof of ownership of a house or condo (e.g., deed or mortgage statement) and utility bill; OR
  2. Statement by a third-party landlord, owner or tenant from whom the parents(s) or persons(s) in parental relation leases or with whom they share property within the district (see NOTE below); OR
  3. Statement by a third party establishing the parent(s)’ or person(s) in parental relationship’s physical presence in the district (see NOTE below).

NOTE: If the parent/guardian is providing a statement by a third-party (as identified in items 2 or 3 above), the third party needs to provide their proof of residency as identified in item 1 above in addition to a statement with their signature notarized. The statement should identify the people residing with them and the conditions under which they are residing there. Additionally, the parent/guardian should provide documents (4 documents recommended), including but not limited to the items listed below, establishing physical presence in the district:

  • Pay stub
  • Income tax form
  • Utility or other bills
  • Membership documents based upon residency (i.e. library cards)
  • Voter registration document
  • Official driver’s license, learner’s permit or non-driver identification
  • State or other government issued identification
  • Federal, state or local agency documentation (i.e. social service agency, federal Office of Refugee)

Proof of residency must include a utility bill. A driver’s license is NOT acceptable as proof of residency.

Custody Papers

If applicable, evidence of custody of the child (including but not limited to judicial custody orders or guardianship papers).

Submitting Forms and Documents

Please fill out pre-registration forms, then scan, copy or take pictures of the required documentation and submit these by mail or email. A free online scanner app is available by clicking here. If you prefer to complete the forms by hand, they are available to be downloaded on this page. 

Once all forms and documents have been digitized, parents should either attach documents to the online registration form or submit their completed packet by email to registrar@jd.cnyric.org or mail hard copies (do not send original documents) to: Registrar, Jamesville-DeWitt Central School District,  PO Box 606, DeWitt, NY 13214.

When the registration process is complete at the district office, you will be directed to the appropriate school building where additional information will be requested, including but not limited to immunization records.

Grades K-12 and PPS Evaluation

To register students in grades K-12 or Pupil Personnel Services (PPS evaluation), parents should go to:

Prekindergarten

The district offers full-day prekindergarten for four-year-olds. Registration for that program will have a separate registration process from grades K-12. Families of eligible students must complete a letter of intent that will appear on the district website in March 2023.

Transportation

It is assumed that the student(s) will be transported, if eligible, in accordance with district policy, from a bus stop near the primary residence as indicated on the enrollment form. If you wish to request (1)  parent pick-up and/or drop off or (2) pick-up and/or drop-off from or to a location other than the primary residence, you must complete a Long Term Alternate Transportation form that can be obtained from any of the school building main offices or the district office.

Requests for transportation to non-public schools must be submitted by parents/guardians in writing on or prior to April 1 of the preceding school year. However, a parent or guardian of a child not residing in the district on such date shall submit a written request within 30 days after establishing residency in the district. Requests must be submitted on an annual basis. A request may be denied for lack of a reasonable explanation, as determined by the J-D Board of Education.
