Welcome to the JDMS Health Office Website!
Stephanie Tortorelli, RN
JDMS School Nurse
And finally, please keep me updated with all changes in your child's health status so that I can better serve his or her needs!
The chill of winter is finally upon us and with that seems to be an increase in sickness. It is our school policy to exclude students from school if they have vomited, had diarrhea or have had a fever (100.0F or higher) within the last 24 hours. Please help keep contagious diseases from spreading by keeping your child home if he or she is experiencing any of these symptoms.
Spring sports will be starting in March. In order for your child to participate he or she must have a physical on file in the nurse’s office dated within one year of March 1, 2016. All physicals must be reviewed for completion by the school nurse, which can take some time. Please turn the physical in prior to the sport’s start date to prevent any delay in your child’s ability to participate.
We are offering school physicals on March 7, 2017. If you would like your child to be added to the list, please fill out and sign the health history portion of the physical form and bring it to the Health Office as soon as possible.
Incoming 7th graders: it is a mandatory year for physicals. A physical is current if dated on or after September 7, 2016. It is also required that your child have a meningitis vaccination.
Incoming 6th graders: A Tdap booster shot is required within two weeks of your child's 11th birthday.
Please turn in all physicals/immunizations records as soon as possible.
